3) PW101122-02: Authorize Mayor and Interim Finance Director to sign Change Order No. 2 to Agreement between the City of Rapid City and Heavy Constructors, Inc. for Deadwood Avenue Reconstruction Phase 1, Project No. 2375, CIP No. 50437.1. This is a no-cost change order, correcting a fund coding error from pay Change Order No. 1.
4) PW101122-03: Authorize Mayor and Interim Finance Director to sign Change Order No. 1F to Agreement between the City of Rapid City and Tru-Form Construction, Inc. for 5th Street Panel Repair – Minnesota to Stumer, Project No. 2657, CIP No. 50445 for a decrease of $36,941.68.
5) PW101122-04: Authorize Mayor and Interim Finance Director to sign Change Order No. 1F to Agreement between the City of Rapid City and Dakota Asphalt LLC for Pavement Rehabilitation Project Chateaux Ridge Court, Project No. 2659, CIP No. 50844.22A for a decrease of $11,400.60.
6) PW101122-05: Authorize Mayor and Interim Finance Director to sign Amendment No. 2 to Agreement between the City of Rapid City and FMG Engineering for Landfill Permit Renewal, Project No. 18-2448, CIP No. 51202. This is a zero-cost amendment.
7) PW101122-06: Authorize Mayor and Interim Finance Director to sign an Agreement between the City of Rapid City and FMG Engineering, Inc. for Sunburst Drive Reconstruction Project No.19-2532 / CIP No. 51253 for Construction Professional Services for an amount not to exceed $26,170.00.
8) PW101122-07: Authorize Mayor and Interim Finance Director to sign an Amendment No. 1 to Agreement between the City of Rapid City and Chamberlain Architect. for Rapid City Fire Station #1, Project No. 2512, CIP No. 51310. This is a no-cost amendment, changing the remaining funding source from 0132 Vision Funds to 8915 CIP Government Buildings.
9) PW101122-08: Authorize staff to advertise for the Pavement Rehabilitation Project – Red Rock Canyon Rd. & Idlewild Ct., Project No. 22-2698 / CIP 50549.22D for $625,000.
